What do you need to do before your visit?
What to Bring to Your Visit
Bringing the right information helps our team check you in and complete your visit more efficiently.
For most visits, bring:
- Photo ID
- Insurance card, if using insurance
- Payment method
- Medication list
- Allergy list
- Any recent test results, if relevant
- Parent or guardian if the patient is a minor
If your visit requires forms, bring:
- School physical form
- Sports physical form
- Camp physical form
- Work or pre-employment physical form
- DOT physical form, if applicable
- Employer authorization or workers' compensation paperwork, if provided
- Travel or immunization records, if relevant
How to Submit Documents Securely
Learn how to submit your insurance card and photo ID before your visit to help speed up check-in. Please make sure all images are clear and readable.

Insurance cards and IDs are protected health information. We're finalizing a HIPAA-appropriate upload portal — encrypted in transit and at rest, with access restricted to authorized MedSurg staff. In the meantime, please do not email insurance cards, IDs, or patient documents. Use one of the secure options below:
Call ahead
Call your preferred MedSurg location and our team will guide you through a secure submission or pre-register you over the phone.
Bring docs to your visit
Bring your physical photo ID and insurance card to your visit. Our front desk will handle everything at check-in.
For your protection, please do not send insurance cards, IDs, or other patient documents via email or unsecured messaging.

Some visits require additional paperwork. Bring whatever form or instruction you received.

Bring any school, sports, camp, work, pre-employment, or DOT form that needs to be completed.
